We pick up right where we left off. Dean, Sam, and Castiel are in Heaven. It IS heaven, right? Paradise?
Spoilers, they don't stay there for long. Basically, the fix-it to end all fix-its, an entire season of typical episodes with additional plot and a bunch of fluff. Eventually, we get there, but it takes a while. They take go hunting, they have therapy, they get their happy ending, though it takes a while.
Episodes aren't being updated for the foreseeable future. Whoops.
Written in script format. I highly recommend you imagine it as if it were actually playing, and definitely listen to all the music as the scenes play out.
