Something was really wrong with Reki.
It took Kojiro longer than he’d like to admit to see that it was worse than he thought.
At first, he brushed Reki’s odd behavior off as some form of coping after his beef with Adam—he couldn’t blame him, Kaoru had acted weirdly after his beef too. Kojiro, Kaoru, and Hiromi (who was, thankfully, out of the hospital at the time) sat Reki down a couple days after the race and gave him the “it’s okay to not be okay” talk (they didn’t want to overwhelm him immediately after the race).
And Reki did what he always did—he smiled cheekily and told them he was fine.
Sure, “fine”. But Kojiro had been a kid once too—a stupid reckless kid who buried himself in skating and covered his feelings with thick fog, so he didn’t push it.

Deku was taken years ago and experimented on with multiple quirks, the villains responsible threaten to add Katsuki's to the mix... unless they can escape together.

Izuku coughs himself awake.
His head hurts. His everything hurts, for some reason, and he reaches out blindly to find the beat-up old phone he keeps on his bedside table. Seven forty-three. He doesn’t need to be up for an hour at least. Sighing, he shuts his eyes and tries to go back to sleep, wrapped in a warm blanket and hugging a spare pillow.
He’s awake again ten minutes later. This time the coughing fit leaves him light-headed and raw, lungs feeling like they’re trying to shrivel up and eject themselves out of his body. His ears seem to be full of cotton. His nose is completely blocked, although sitting up makes it marginally less awful to breathe. The back of his head throbs like his brain wants to burst out of his skull.
Oh. I’m sick, he thinks dimly.
(Izuku, a tired omega, is sick again from overwork. His boyfriend Katsuki is here to help.)

Midoriya trembles for another moment before dropping roughly to his knees.
Shouto is at his side in an instant. “Midoriya,” he says, placing a tentative hand on his shoulder and searching his face. Midoriya is still staring straight ahead at nothing, but now his lips are slightly parted. Shouto couldn’t tell from the street, but up close he can see that Midoriya’s breaths are shallow and rapid, like a frightened rabbit. He’s hyperventilating.
“Midoriya. What did she do? Did she use her quirk?” Shouto rakes his eyes down his friend’s body, searching for any sign of injury. He looks fine, aside from-
A violent shudder erupts through Midoriya’s body. His head lolls forward and Shouto has to grab him around the torso to keep his friend’s limp body from hitting the concrete.

Or, during a villain encounter, Midoriya is hit with a virus quirk that causes his body’s immune system to turn on him. He gets really sick really fast. Todoroki takes care of him while learning how exactly to comfort another human being and realizing that he is extremely touch starved.

Izuku thinks the words should make him upset, but they don’t. He’s too busy digging his nails into his legs, thinking about the room, to be upset about anything Kacchan is saying.
“At the end of the game, do … Do you ever go into a room with them? With your parents’ friends and ... do stuff?”
When he turns around, Kacchan is staring at him with a twisted expression.

He was too afraid to move. Too afraid to breathe, as the illusion enveloped him. Ashido’s constant chattering faded away. The weight of Aizawa’s watchful gaze lifted from his shoulders.
“Izuku,” The desk. The chair. The TV in front of him. The scent of blood. His own blood. The sound of life support, the beeping and mechanical breathing. “Sit down.”
He was sitting on the chair suddenly, hyperventilating. Fear clogged his airways, as his father took his time walking around the desk. A black notebook and a pencil sat on the desk now, but he didn’t reach for them. Izuku knew not to reach for them. His back was littered with scars from the last time.
His father stopped right behind him and put his hands on the back of the chair, his knuckles digging into Izuku’s back. His entire body tensed. His father leaned down to whisper in his ear, and a remote materialized from his hand. A video began to play.
“Tell me what you see, my boy,” All for One smiled. “You have one chance. Sixty seconds. Tell me their Quirks. Tell me how to kill them,"

Summary
Izuku had a soulmark before. Explosions across his collarbone, bright and loud, a neon sign that screamed for all the world to hear exactly who his soulmate was.
It disappears in his final year of middle school. Izuku asks the universe to take it away after his last straw is taken in Kacchan’s hands and snapped in half, and he isn’t granted a second one. Not that he minds all that much- no, no one is better than Kacchan was, and Izuku thinks he can settle for finding love one day without the help of some fancy mark on his skin.
Hitoshi never had a soulmark before. He grew up without one and got bullied for it. After all, he was the villain kid, so despicable with such an evil quirk that even the universe agreed he wasn’t worthy of love. He grew to accept it slowly, eventually coming to terms with the fact that he’d have to find someone on his own even if the odds were stacked against him.
Both of them are surprised one night to see a splotch of color on their hands, a thin matching thread that begins to grow as the days go by.
Thankfully, neither are disappointed.
