Rem had purposefully danced around the subject, only telling him that boys and girls were built different.
"Just like Plants are different." she'd said, giving Nai that smile which never quite reached her eyes.
Nai had the feeling there was more she wasn’t telling him, but he'd let it go for the time being. They were Plants after all, and while they might look human, they weren't. That was something Nai had to remind Vash of. Too often for his liking.
"Did you let Rem see?" Nai asks, unsure why he hopes Vash's answer will be no.
Vash nods. "She said I should try and touch myself. But I didn't want to. I was scared." He grips Nai's shirt, "I’m scared, Nai. What if there's something wrong with me?"
Nai takes Vash's hands in his, intertwining their fingers as he reassures, "There's nothing wrong with you Vash. If there were then something would be wrong with me too, right? What does Rem always tell us? Twins are the same."
"Twins are the same," Vash echoes, relaxing slightly.

In the aftermath of their final fight, Vash washes his brother's hair. The two of them put each other back together--slowly. It gets worse before it gets better.

Knives’s hair is short, prickly—but so pale that when he moves his head and the light hits it just so, it looks almost translucent. When he was younger—in those eighty years they spent together, keeping only each other’s company—Vash had thought it made his brother look like he wasn’t made of anything mortal. Like he wasn’t made of dust and dirt, not like everyone else.

Summary
From that fateful night the boy had suddenly showed up in Conrad's study years after the Big Fall, worrying about him more or less came as second nature. It's not that he didn't think Knives could hold his own on a day-to-day basis - it was the accumulation of things, the slow and ominous snowballing as, time and time again, just like today, Knives shouldered his concern aside and plowed onward.
Conrad had seen it all before. That's why, even as the powerlessness and guilt ate at him relentlessly, there was never a time he wished he could forget it. It served as a warning, one which Conrad vowed never to ignore; After all, it could very well be Knives' only hope of surviving the inevitable crash.
OR: A young Knives learns the hard way that there are some things he can't deal with on his own. Fortunately, learning he doesn't have to comes a little more easily.
