“Kacchan,” Izuku whispered into his neck. He gripped Kacchan’s shirt desperately, like it was the only thing keeping him grounded. “I can’t keep p-pretending.”
“What do you mean?” Kacchan murmured.
“I can’t keep pretending that I don’t want you,” Izuku whimpered, his whole body trembling. “That I don’t need you.”
Or, Izuku puts distance between him and Katsuki because it hurts to be around him when he knows they can never be together. Katsuki notices and confronts him.

Izuku's arm felt heavy as he lifted it. He placed his palm against Katsuki's chest. His fingers shook as he laid them flat, feeling the soft thud of his heartbeat. “You're alive.” For the first time that night, Izuku took a deep breath, his eyes fixed on the way his hand rose and fell with every inhale.
“Of fuckin’ course I am, you idiot.” Despite the harsh words, Katsuki's voice seeped straight into Izuku's veins, and instant relief washed over him.
‘Of fucking course he is.’ He thought.

This takes place a few months after they fight the war. There will be some minor manga spoilers.
