Summary
This is a “Ripple Effect” fix.
While I love the hug that Sam and Janet get at the end of that episode and while I sprinkle that far and wide across the interwebs using the gif I obviously had to create of that wonderful scene… my question about that episode has always remained:
Zero interaction other than that hug, really? Really!?
So, I rewrote the episode and laid out a little pre-episode S/J backstory to get us off on the right, lovey-dovey, flirty foot.As for the rating: there is an explicit chapter. There is also mention of domestic violence / rape having occurred in one character's past.

Summary
When Maya and Carina began IVF they purchased a 5 year plan to have their embryos frozen. After Andrea was conceived on the first round, three frozen embryos remained.
The 5 years have now come to an end, and the Deluca-Bishops received a letter requesting they decide what they’d prefer done with their remaining embryos. Maya and Carina had previously discussed donating them – either for another family to use, or for research. But, Maya’s been considering a different, unspoken possibility. Since Andrea was born she’d wrestled alone with thoughts of suggesting having a third baby.
