Summary
Everyone knows the story of 'One for All' and 'All for One'. Their bout went on for generations passing to different user, growing in power, reaching Singularity with their 9th and 3rd users respectively.
Due to the 'Meta' nature of the brothers Quirks they have full access to the 'Vestige Realm', a mental space where Vestiges primarily manifest.
In their "final clash", the Quirks were "Destroyed" spontaneously releasing decades of raw energy, exploding out and permeating through every human on the Planet.
Permeating the Human race, the Vestige Realms expanded, creating a Cognitive World born from the collective Superhuman psyche.

Summary
Admist the "Final War", Midoriya finally defeats Shigaraki with the embers of One for All. As it turns out, the petty bastard All for One is, had a final trick, a portal to banish Midoriya.
This sends an nearly unconscious Izuku onto a trapped Yuno after he was captured by the Chain Villian, Revchi. Tired and fatigued, Midoriya, still barely able to use OfA attempts to save him.
