Summary

“… You are insane if you think I’m going to come with you so willingly.”
Holmes’ face feigned to a saddened look as his eyebrows scrunched and his lips shook. It almost made John feel a small pain of guilt as it wasn’t as if this was his favorite work and he was paid well, he just didn’t prefer the idea of being aboard something possibly more abysmal than what he had already experienced. That guilt quickly drained from his body as Holmes leaned in close to his ear, his hushed words causing John to strain to hear him over the violence around them. He’d later wished he hadn’t.
“It’s a shame then that I wasn’t asking.”
John's ship is boarded by the Pirate Sherlock Holmes and he is willingly (more or less) offered a job aboard their dreaded ship 'Redbeard' as their new doctor.

A random offhand remark causes Sherlock to research the Foster Care system, which he becomes very interested in. They decide to apply to be foster parents and their first placement is a trans teenager named Max.
