Summary
Without Loki and Tony Stark, Thanos inevitably won. His first snap culled half of the universe, and his second snap destroyed the Infinity Stones.
However, the Infinity Stones were never mere tools or singularities. They were siblings, always connected and always separate. They existed as a fundamental paradox unto themselves, able to work in tandem but without truly interfering. This all changed when Thanos forcibly destroyed the Stones, scattering their power throughout the universe.
As the years passed, the siblings began to lose themselves. The separation was never meant to be severed, but Time especially relied on the separation to keep the multiverse stable. With the Titan’s actions, everyone that remained was still at risk. But how could they fix it? If Time simply reversed the clock, everything would fall apart just as it had before. So what could the Infinities do?
Well, it required a reluctant partnership.

Loki and Tony Stark get sent back in time by the Infinity Stones to stop Thanos. Can they learn to work together for the sake of the universe?

Summary
Maybe, if Dumbledore hadn't been so quick to sweep Harry's first murder under the rug, Harry might have thought on it hard enough to feel regret. Instead, Dumbledore, as a trusted authority figure, assured the young boy saviour that he did no wrong.
Had Dumbledore been in any way competent at child rearing as opposed to training child soldiers, Harry would not have ended up making an accidental horcrux. Voldemort would still be insane. Nagini would have no hope of ever having grand-hatchlings to dote on.
(Harry somehow winds up in the middle of a ritual circle just as Voldemort is finishing the ritual to make another horcrux. Voldemort tries to backpedal. All he succeds in is becoming Harry's horcrux. Which might have been reversible had Harry not been harboring a secret in his scar.)
