Of course Sam is the one who figures it out.
"God, Dean, you just have to touch everything, don't you," he's bitching, gesticulating pissily over the basket of brackish-stained laundry he just dumped on Dean's bed. On top of the pile is the neat carved figurine Dean found in the antique shop. Dean's not sure what this has to do with Cas yet, but apparently it went through the wash in his pants' pocket and now a bunch of Sam's flannels are ruined? That's what he's gleaned so far.

Summary
In the game of a lifetime, Castiel’s amateur team is allowed to play against one of the nation’s best teams in the national cup.
Which means Castiel faces Dean Winchester, the super star of the league — and tackles him. Everything happens so fast, but he can hear the destruction he causes when Dean goes down.
In shock, Castiel doesn’t care about being sent off. All he cares about is Dean. Solely focused on Dean’s wellbeing, he follows the medics as they carry Dean off the field and towards a helicopter.
Despite his own agony, Dean notices Castiel’s condition, and doesn’t hesitate. He persuades the medics to take Castiel with them.
When Castiel later awakes in a hospital room, Dean is already waiting for him.
