Summary
Dr. Gurathin arrives on Preservation and keeps running into trouble when he tries to process the world around him with the mind of a Corporate. They all keep telling him "Gura, that's not how hospitals work, Gura, that's not how friendship works, oh Deity Gura please that's DEFINITELY not how sex works," but it won't get any easier to swallow. He knows intimately that debt can never really be forgiven, someone always needs to eat the cost, but why does no one else seem to notice?

"Can we talk about what happened?" Mensah said, settling down onto the couch.
"I suppose." Gurathin had wrapped his arms around his stomach. Classically defensive. He was a walking cliche. "Are you mad?"
Gura and Mensah have a talk. Fits in somewhere in the middle of Chapter 4 of A Patch of Snow on the Ground, probably doesn't make sense alone.

Summary
Watson inevitably worried a great deal when Holmes got into this state, no matter how frequently it happened or how familiar this pattern was. He knew to expect Holmes to forgo sleep while on a particularly stimulating case, to be so full of energy and brimming with restless agitation that most sleep would be just the occasional nap. It did not happen on every case to this severity, thankfully, but there was always at least some impact if the case was urgent.
In this instance, the case was very urgent, and success necessary in order to prevent a war.
