Summary:

I am uable to name the originator of the codenames of "Book" and "Bolt", but if they will contact me, I will edit to acknowledge their intellectual property.

Tales That Are Told About "Bolt and Book"

They are known to be a mysterious duo, in whatever branch of the multiverse they pop up in.

A Man and Woman Team, assumed, but not proven, to be man and wife.

In branches of the multiverse that have mutants, they are assumed to be mutants.

In branches of the multiverse which have "capes", though many don't wear them, they are assume to be of the class of "capes".

In the branches of the multiverse where witchcraft and magic are real, they are assumed to be a witch and a wizard.

Do you see a pattern here? 

They are known to few, and how they do what they do is speculation.

They are known to have an associated couple, another team, who are code named Butterfly and Fluttterby.  How the two teams are connected is unknown.  Who the other team 's identities are is unknown, too.

In magical 'verses, it is believed their identities are known to no more than six people, and is under 'fidelius' with them.

Much of what is said about them is speculation, some bits are fact.

Book, for example, is said to have a library containing copies of every book in the Hogwarts Library, the British Museum library, and the US Library of Congress.  If true, no one talking has ever seen it.

Bolt is alleged to have acquired his codename by his preference for "lightening" charms, and many they are suspected of hunting down are found dead with electric burns on them.  But no one knows for sure who gave him that moniker or why.

They seek no media coverage of their activities, and are seldom photographed or recorded, such devices suddenly failing when they are present.

On very rare occasions they "sign" their work, usually by leaving a business card bearing a picture of a book with a lightning bolt on the cover.

Some of the circumstances where they strike appear to show they have some level of control of time travel, and are not afraid to use it.

They are speculated to have their 'hideout' in the Pacific Northwest, Alaska, or somewhere under the Rocky Mountains, presumably hardened enough to make 'Cheyenne Mountain' look like a child's sand castle.  This is based on the fact that the far west  of North America is where they have most often appeared.

Bolt and Book seem to be operating above the law everywhere they work, as no LEO organization has ever been authorized to arrest them, no warrants issued, if they could be found to arrest.  This may be urban legend, as they are as easy to find and hold as a glass of fog, or like trying to catch smoke in a net.
