Commander Shepard did not expect to find another person with the condition on board. Osteogenesis Imperfecta was not a very common condition in all of humanity, but Shepard believed Joker when he said that he had it.
It was written on his face. In the greyness of his teeth and the blue tint in the whites of his eyes. In the way their facial structure matched perfectly, like they could fool someone into believing they were related.
It was a shock, because Shepard never expected to meet someone else with the same brittle bones that made up the commander's adolescent body. They had strengthened since, of course. Shepard only had Type 1 Osteogenesis Imperfecta, after all. The type to pass after adulthood. The type to get them picked on during Never Have I Ever games. The type to get them out of P.E.
Not the type to make every moment miserable.
One day, after they catch Saren, Shepard figures that the secret will come out and they'll talk. Maybe it'll be before that, since Dr. Chekwas could easily match the symptoms to the disease if she got access to the commander's files. But either way, one day the soldier and the pilot will sit together over glasses of wine and exchange stories, and maybe it'll slip out that Joker isn't the only one on board with Vrolik's syndrome.
Maybe one day.
